Job Description

The Social Worker would work Monday-Friday 7am-3pm or 9am-5pm full-time in Queens, NY.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Supervise a team of Case Managers.
• Hold regular, structured supervision meetings with direct reports.
• Promote effective case management strategies using Housing First, Harm Reduction, Crisis Prevention Institute (CPI), Motivational Interviewing, and trauma-informed care models.
• Develop and implement individual service plans, ensuring comprehensive and timely documentation of client contact and progress.
• Conduct interviews and assessments of potential clients.
• Provide clinical direction and leadership to the program, assisting with troubleshooting, problem-solving, and strategizing solutions for client engagement and challenging behaviors.
• Foster a staff and resident culture that emphasizes housing placement as a key indicator of success, ensuring clients are prepared for housing placement.
• Oversee the development and implementation of individual service plans, ensuring thorough and timely documentation of client interactions and progress.
• Assess and evaluate client functioning.
• Supervise and facilitate case conferences.
• Coordinate care delivery with multiple service providers, particularly outreach teams.
• Collaborate with outreach teams to ensure individuals succeed in transitional housing and prevent a return to homelessness.
